Medicare and Taxes

You’ve probably heard the old saying: There are only two certainties in life — death and taxes. But did you know that one of those certainties (taxes) can help you get the medical care you need to live healthier? Today, we’re exploring the world of Medicare taxes and how tax dollars you contribute now can offer you peace of mind and medical protection in the future.
Please note: This is not tax advice. We are providing a basic overview on how Medicare taxes work for educational purposes only and not to inform any financial decisions.
What is the Medicare tax?
The Medicare tax is a deduction taken from your paychecks to pay for Medicare Part A, which covers hospital and emergency care. It’s a federally required payroll tax and is automatically deducted from your paycheck.
How does the Medicare tax work?
The Medicare tax is paid in two parts. You pay a portion of it as a withholding from your paycheck and the other part is paid by your employer. This happens automatically as part of the payroll process. Your employer is required to collect the tax, and it sends funds from both the employee and employer to the IRS through regular electronic deposits.
People who are self-employed also pay a Medicare tax as part of their self-employment tax. Rather than being withheld from a paycheck, it’s paid through quarterly estimated tax payments.
What is the Medicare tax used for?
The Medicare tax is used to fund Medicare Part A. Medicare Part A provides health insurance for those age 65 and older as well as people with certain disabilities. Medicare Part A is the part of Medicare that covers hospital insurance. It includes health care costs for inpatient hospital stays, skilled nursing care, hospice and some home health services.
Medicare taxes account for approximately 88% of the total revenue for Medicare Part A, which is about $284 billion. The rest is paid for by income taxes paid on Social Security benefits and funds authorized by Congress for Medicare use.
What’s the current Medicare tax rate?
In 2024, the Medicare tax rate is 2.9%. For most people, that typically means that you pay 1.45% and your employer pays 1.45%. People who are self-employed pay the full 2.9% themselves, and individuals with taxable income of over $200,000 pay an additional 0.9% per year.
Are all wages subject to Medicare tax?
All taxable employment earnings are subject to the Medicare tax. This includes salaries, overtime, paid time off (PTO), tips and bonuses. There is no maximum on the amount that can be taxed. That’s different from the Social Security tax where you only pay on the first $168,600 of income.
The tax is calculated based on your Medicare taxable wages. It is determined by starting with your gross pay and subtracting any pretax health care deductions, such as medical insurance, dental, vision or health savings.
Why is the Medicare tax higher for self-employed individuals?
Technically, the Medicare tax is the same for everyone. However, it seems higher for self-employed individuals because they don’t have the benefit of their employer paying half of the tax. Instead, it is up to the self-employed person to cover the full amount of the tax, which is what makes it seem like they’re paying twice as much.
Do high-income earners pay more Medicare tax?
The Additional Hospital Insurance Tax, which is more commonly called the Additional Medicare Tax, is part of the Affordable Care Act (ACA). It’s designed to help support both Medicare and the ACA. Only those with annual incomes over $200,000 are subject to this tax, which is currently 0.9% of eligible wages. Money that comes from this tax is used to offer more affordable health care to over 9 million Americans nationwide.
Conclusion
The Medicare tax affects everyone who earns an income in the United States. It funds important health care coverage for older Americans, as well as people with certain health concerns and disabilities. For most, the tax is removed automatically as part of the payroll process. And the tax makes it possible for Medicare Part A to offer health care coverage to millions of people nationwide.
